(DHM), the management company for Digestive Health Associates of Texas P.A (DHAT), is responsible for the staffing, billing and collections, and managed care contracting and credentialing for all of the DHAT physicians and endoscopy centers. When are you eligible to enroll?
If you are a full-time employee, working thirty (30) or more hours per week, you are eligible to enroll in our benefits program on the first day of the month coinciding with or next following your completion of sixty (60) days of employment.
Who is eligible to enroll?
Your legal dependents include your legal spouse, domestic partner, natural and adopted children, stepchildren, children of a domestic partner, and any other children you support for whom you are the legal guardian, or for whom you are required to provide coverage as the result of a qualified medical child support order. Eligible children may be enrolled in benefits up to age twenty-six.

FLEXIBLE SPENDING ACCOUNTS
- Health Care Flexible Spending – up to $2,600 tax-free annual election – with up to $500 annual rollover

HEALTH SAVINGS ACCOUNT
If you select the Consumer Driven Health Plan, you may set money aside on a tax-free basis to pay for eligible medical, dental, and vision expenses. There is a maximum annual contribution set by the IRS. For 2016, those maximums are $3,350 if you have Employee Only coverage; and $6,750 if you have Family coverage; this includes Employee and Employer contributions to the Plan. DHAT will contribute the following non-taxable amounts to your HSA on a quarterly basis:
- $150 per quarter if you elect Employee Only coverage
- $250 per quarter if you elect Employee plus one or more dependents coverage
Unlike a Health Care Flexible Spending Account, that has stricter rules, your HSA funds accumulates indefinitely, so anything left at the end of the year rolls over to the next year. If you change jobs or retire, you can take your account balance (including the Employer contribution) with you and use it to pay for future medical, dental, and/or vision eligible expenses.
LIFE INSURANCE/ ACCIDENTAL DEATH & DISMEMBERMENT
- Company-paid Basic Life Insurance and AD&D
- Employees may purchase additional employee and dependent Life Insurance and AD&D
DHAT 401(K) RETIREMENT AND PROFIT SHARING PLAN
For purposes of salary deferral contributions and ROTH deferral contributions, you may become a participant on the first day of the month coinciding with or next following your completion of sixty (60) days of service and your 18th birthday.
DHAT will make a matching contribution on an annual basis equal to 100% of the first 3% of your pre-tax or ROTH salary deferral contributions, plus 50% of the next 2% of your pre-tax or ROTH salary deferral contributions up to a maximum of 5% of your salary. Employees are always 100% vested in Employee contributions, Employer Safe Harbor Match contributions, Rollovers, and any earnings on these contributions.
DHAT may choose to make an additional annual non-elective contribution. If so, the Employer’s contribution will be allocated among the accounts of all eligible participants. Employees become fully vested on non-elective Employer contributions after three (3) years of service.

PAID TIME OFF
Vacation Paid Time Off is available to Regular Full-Time employees. Eligibility and PTO Plans may vary for Staff, Directors, Mid-Level Providers, and Physicians.
Illness Paid Time Off is available to Regular Full-Time employees. You will begin accruing illness paid time off on the first day of the month coinciding with or next following your completion of sixty (60) days of employment.
Floating Holiday Paid Time Off is available to Regular Full-Time employees. You will receive one (1) floating holiday per calendar year. The floating holiday must be used within the calendar year in which it was granted; and a new one is granted on January 1st of each year, to eligible employee.
